Plan (CDPHP) is a health maintenance organization serving upstate
NY and Vermont.
It has more than 400,000 members in 29
New York counties: Albany, Broome, Chenango, Clinton, Columbia,
Delaware, Dutchess, Essex, Franklin, Fulton, Greene, Hamilton,
Herkimer, Jefferson, Lewis, Madison, Montgomery, Oneida, Orange,
Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, St. Lawrence,
Tioga, Ulster, Warren, and Washington, as well as Addison, Chittenden,

CDPHP was founded in 1984 as a not-for-profit
Individual Practice Association (IPA) model Health Maintenance
Organization (HMO) in Albany.

York, Medicare Choice (group and individual), Medicaid, Child
Health Plus, and Family Health Plus.
CDPHP Universal Benefits, Inc. - Preferred provider organization (PPO); High Deductible
PPO (HDPPO); exclusive provider organization (EPO), and point-of-service
(POS).
Capital District Physicians' Healthcare
Network, Inc. - Administrative services organization and self-insured plans.
APA Partners, Inc. - third party administration (TPA).

HEALTH INSURANCE
· AttentiCare EPO - $20 co-pay per visit
· Includes prescription coverage
· Coverage begins the first of the month following date
of hire
· Deductions on a pre-tax basis
· Dental and vision coverage available separately
OR
· HSA High Deductible PPO with HRA - $1,500 individual
or $3,000 family deductible
· 10% coinsurance after deductible is met
· HRA funded by CDPHP - $1,250 individual or $2,500 for
family coverage
· Prescription coverage with copay after deductible is
met

LIFE INSURANCE / ACCIDENTAL DEATH &
