The Capital District Physicians Health Plan (CDPHP) is a health maintenance organization serving upstate NY and Vermont.

It has more than 400,000 members in 29 New York counties: Albany, Broome, Chenango, Clinton, Columbia, Delaware, Dutchess, Essex, Franklin, Fulton, Greene, Hamilton, Herkimer, Jefferson, Lewis, Madison, Montgomery, Oneida, Orange, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, St. Lawrence, Tioga, Ulster, Warren, and Washington, as well as Addison, Chittenden, Franklin, Lamoille, Rutland, Washington, and Windham counties in Vermont.

With headquarters in Albany, CDPHP has regional offices in Highland, NY and Syracuse, NY.

CDPHP was founded in 1984 as a not-for-profit Individual Practice Association (IPA) model Health Maintenance Organization (HMO) in Albany.

The CDPHP family of products includes more than 9,000 providers and practitioners under four business lines:

Capital District Physicians' Health Plan, Inc. - HMO, Healthy New York, Medicare Choice (group and individual), Medicaid, Child Health Plus, and Family Health Plus.

CDPHP Universal Benefits, Inc. - Preferred provider organization (PPO); High Deductible PPO (HDPPO); exclusive provider organization (EPO), and point-of-service (POS).

Capital District Physicians' Healthcare Network, Inc. - Administrative services organization and self-insured plans.

APA Partners, Inc. - third party administration (TPA).

Employee Benefits

HEALTH INSURANCE
· AttentiCare™ EPO - $20 co-pay per visit
· Includes prescription coverage
· Coverage begins the first of the month following date of hire
· Deductions on a pre-tax basis
· Dental and vision coverage available separately
OR
· HSA High Deductible PPO with HRA - $1,500 individual or $3,000 family deductible
· 10% coinsurance after deductible is met
· HRA funded by CDPHP - $1,250 individual or $2,500 for family coverage
· Prescription coverage with copay after deductible is met
· Coverage begins the first of the month following date of hire
· No cost to employees
· Dental and vision coverage available separately

LIFE INSURANCE / ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE
· Life Insurance equal to 112 times annual salary
· Accidental Death & Dismemberment at 112 times annual salary
· Coverage begins the first of the month following date of hire
· Additional coverage for employees and their family is available at cost

FLEXIBLE SPENDING ACCOUNT (FSA)
· Set aside money on a pre-tax basis to cover dependent care or unreimbursed medical expenses

401(k) PLAN
· Eligible to enroll on date of hire
· Employees can contribute a maxium of 75% of earnings, up to the IRS contribution limit for the plan year
· Company match after 6 months of service
· Fully vested after 3 years
· 401(k) loans and hardship withdrawals available

PROFIT SHARING (defined contribution plan)
· Eligible after 6 months of service
· Fully vested after 3 years
· Variable Company contribution

UNIVERSAL LEAVE (Combined vacation, sick, and personal time)
· Salaried Employees:
· Less than 3 years of service accrue 22 days per year
· 3 - 9 years of service accrue 27 days per year
· 10 + years of service accrue 32 days per year
· Hourly Employees:
· Less than 3 years of service accrue 17 days per year
· 3 - 9 years of service accrue 22 days per year
· 10 + years of service accrue 27 days per year
· Carry forward or cash out option at end of year

HOLIDAYS
· 10 Paid Holidays (includes 1 floating holiday)

SHORT-TERM AND LONG-TERM DISABILITY
· Both Company paid
· Additional insurance available at cost
